Refer a patient

Information to assist health professionals referring patients to our services

Refer a patient

The following information is to assist general practitioners (GP) and private specialists referring patients for services at either Fiona Stanley Hospital or Fremantle Hospital.

Urgent referrals

Please phone the registrar or consultant of the specialty team at the relevant hospital by phoning the:

  • Fiona Stanley Hospital Helpdesk on 6152 2222
  • Fremantle Hospital switchboard on 9431 3333.

Contacting a registrar or consultant ensures the specialist team:

  • is aware of the referral immediately
  • can make arrangements including interim treatment recommendations and further investigation prior to review as soon as possible.

Fiona Stanley Hospital urgent referrals

Once the urgent referral to Fiona Stanley Hospital has been accepted, fax to 6152 9762 or email Fiona Stanley Hospital Referrals.

Emailed referrals must be an attached PDF (a PDF is a secure document).

Please note on the referral the name of the doctor with whom the referral was discussed.

Fremantle Hospital urgent referrals

Unless instructed to fax directly to the accepting doctor, when sending an urgent referral to Fremantle Hospital please:

  • mark the referral as 'urgent'
  • include on your referral the name of the doctor you spoke with, any instructions given or any appointments made
  • fax the referral and attached reports to 9431 2009 or email Fremantle Hospital Central Receipting Clinic, ensuring the fax or email header clearly identifies:
    • patient name
    • clinic referred to
    • appointment date
    • the total number of pages sent if faxing.
Fiona Stanley Hospital direct referrals

The following referrals are accepted directly to Fiona Stanley Hospital and should not be sent to the Central Referral Service:

  • referrals for patients who require immediate review (within next 7 days)
  • obstetrics services referrals
  • mental health services referrals
  • Rapid Access Chest Pain Clinic referrals.
Non-urgent GP and private specialist referrals

For Fiona Stanley and Fremantle hospitals

All non-urgent GP and private specialist referrals to doctor-led outpatient clinics at either Fiona Stanley or Fremantle hospitals should be sent to the WA Health Central Referral Service (external site).

  • Fax: 1300 365 056
  • Postal address: GPO Box 2566, St Georges Terrace, WA 6831

Referrals are accepted on the letterhead of the GP, generated from your practice software or using an online referral form templates (external site). The following patient information is required:

  • date of birth
  • address and contact phone numbers
  • Medicare number (including expiry date)
  • patient’s maiden name
  • language requirements (interpreter)
  • current medication and allergy information
  • weight (particularly if over 120 kilograms)
  • copies of relevant investigations and reports
  • GP contact details.

Priority

Referrals are clinically prioritised as:

  • urgent: priority 1 (0 to 30 days)
  • semi-urgent: priority 2 (31 to 90 days)
  • routine: priority 3 (91 to 365 days).
