Information for GPs and healthcare professionals

Information for GPs

The following is information for general practitioners (GPs) and other health professionals who refer patients to the Fiona Stanley Fremantle Hospitals Group.

Referrals

Read about referring a patient for services at Fiona Stanley or Fremantle hospitals.

You can also read specific referral information for some services.

Fiona Stanley Hospital contact list for GPs

Our General Practitioners contact list (PDF 228KB) is available to assist GPs with patient referrals to Fiona Stanley Hospital.

In the instance where an urgent or inpatient referral is required, GPs can contact the relevant Fiona Stanley Hospital speciality for further advice and guidance.

Obtaining medical records

Please note that much of this information is now being uploaded to the patient’s My Health Record and may be accessible in this way.

For Fiona Stanley Hospital patients

Please either send a medical information request on letterhead via fax 6152 4041 or email Fiona Stanley Hospital HIMS Health Records to obtain the following records:

  • discharge summaries
  • outpatient letters
  • procedure reports (for example, gastroenterology, cardiology, neurology, respiratory).

If the request is not from the GP recorded by the patient on our patient management system, then a breach of patient confidentiality exists. In such circumstances, as per Department of Health policy, the GP practice should also provide a copy of the patient's consent to verify the patient approves this GP accessing their personal records.

For Fremantle Hospital patients

Please send a medical information request on letterhead to medical records fax 9431 3685 or email Fremantle Hospital Health Information Management Service.

If the request is not from the GP recorded by the patient on our patient management system, then a breach of patient confidentiality exists. Obtaining radiology results

Medical imaging results may also be accessed via the patient’s MyHealthRecord

For Fiona Stanley Hospital patients

Please send a written request on practice letterhead to Radiology on fax 6152 4941.

For Fremantle Hospital patients

Please either:

  • ring Fremantle Hospital Radiology on 9431 2301
  • fax a request on letterhead to 9431 2663
  • email Fremantle Hospital Radiology.

If the imaging was requested by a GP referrer in the community the result will be sent via Healthlink secure messaging.

Obtaining pathology results

Most pathology results may also be accessible via the patient’s MyHealthRecord

For Fiona Stanley Hospital and Fremantle Hospital patients

Please phone PathWest on 13 72 84.

Advise receipt of incorrectly directed patient information

For Fiona Stanley Hospital patients

Please return by fax to Medical Records PMI clerks on fax 6152 4041.

For Fremantle Hospital patients

Please return by fax to 9431 3685 or email Fremantle Hospital Health Information Management Service.

GP Engage events provide valuable opportunities for local primary care professionals such as GPs and nurse practitioners to learn about Fiona Stanley Fremantle Hospitals Group priorities, processes and programs affecting shared care.

GP Liaison

Fiona Stanley Hospital works closely with a range of health professionals including general practitioners, allied health practitioners, nurses and medical professionals to provide excellence in service delivery and care for patients.

Dr Monica Lacey is the Fiona Stanley Fremantle Hospital’s Group GP Liaison who can be contacted for non-clinical enquiries and concerns.

The GP Liaison role is focused on promoting clear and relevant communication between the hospital and primary care providers, particularly in the areas of patient referral and discharge.

The position also enables the representation of the general practice point of view during the development of hospital and health service policies and processes which affect shared patient care.
